    • Job Requirements
    • BLS certification from the American Heart Association is mandatory
    • License/Certificate Verification must be performed within 30 days of start
    • Fit Test is required prior to onboarding
    • NIHSS certification is necessary
    • Trinity 9 Panel Drug Screen must be completed within 30 days of start, preferred within 14 days
    • Trinity Code of Conduct Form must be on file
    • Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement must be signed
    • Trinity Integrity and Audit Services Form must be on file
    • Trinity Specialty Specific Competency Testing must be completed within a year
    • Trinity Non Employee Upload Form must be fully completed and uploaded
    • Trinity Skills Checklist has to be provided by clinical contractors
    • 2 Step PPD OR IGRA test is compulsory
    • Trinity Specific Covid Questionnaire is to be completed
    • COVID-19 Vaccine documentation required (proof, declination, or exemption)
    • MMR vaccination verification is necessary
    • Hepatitis B immunization series should be started prior to start date
    • Varicella vaccination or immunity proof is required
    • Tdap vaccination documentation needed unless with a valid declination
    • Physical/Health Statement confirming fitness to work is a prerequisite for onboarding
    • Resume, Skills Checklist, and 2 Professional References are required for Trinity Profile
