    • American Heart Association certification is required for all direct patient care personnel.
    • Must include an image of the front of the card as well as an image of the back of the card.
    • Must be dated within 30 days of start.
    • 9 Panel must be completed and resulted before start with MRO signature.
    • All engaged contractors must have page 27 signed form on file in Trinity VMS.
    • Standard i9 completed by the agency.
    • All engaged contractors must have page 5 signed form on file in TrinityVMS.
    • Attach copy of specialty exam must be completed within a year.
    • Proof of vaccine, declination or exemption(with agency letterhead). Declination not accepted for CA or IL.
    • Attached scanned proof of 2 vaccinations with dates or primary source IgG titer with lab print-out to candidate's record in Trinity VMS.
    • Must have 1st vaccination in series prior to start; 2nd shot in series 30 days later; 3rd shot in series 6 months later.
    • Proof of vaccine, declination or exemption with agency letterhead for Influenza Vaccine.
    • Resume, Skills Checklist within a year, 2 Professional References.
    • All engaged contractors must have page 1 signed form on file in TrinityVMS.
    • Federal, Nationwide, County Criminal from last 7 years including SSN Verification and Employment verifications, dated within 30 days of start.
    • Attach completed 3rd Party Screening Company report of candidates highest level of academic education.
    • Attach scanned EPLS report from website or result provided by third party screening company dated within past 30 days.
    • Attach scanned OIG result print-out from the website or result provided by third party screening company and dated within past 30 days.
    • Must be dated within 30 days of start. Check all names used and locations.
    • Must be completed and uploaded one week before start.
    • Iowa's Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required.
