Job Description:
The ADON- Admissions Nurse is responsible for oversight of all
aspects of the admission process including initial assessment and
care plans along with ADON duties. This position is responsible for
establishing and maintaining open, effective communication between
facility staff, referral sources, family members, physicians and
applicable payer sources. Essential Functions and
Responsibilities:
- Collaborating with facility Community Relations Representative,
identifies and maintains relationships with hospital discharge
planners. Fosters relationships with insurance providers, referrers
and physicians.
- Receives and/or reviews referral from physician, hospital, or
insurance-based case manager on all transitional or subacute
patients.
- Responds to inquiries from hospital discharge planners related
to possible admissions. Reviews assessment for appropriate
placement.
- Collaborates with DON/MDS in conducting telephone and faxed
copy admission assessment to ensure appropriate clinical admission.
May conduct on-site evaluation if necessary.
- Coordinates and arranges for all equipment and supplies needed
upon admission.
- Ensures initial and ongoing fiscal appropriateness of resident
admissions to nursing facility.
- In conjunction with DON and therapies, ensures the
establishment of a treatment plan that is timely, cost-effective
and based on prioritized objectives.
- Monitors resident progress and communicates information between
patient, referrer, family, insurance providers and facility nursing
team.
- Assists with coordinating daily and weekly interdisciplinary
resident care conference for SNF residents.
- Provides constructive influence and input regarding treatment
program development and enhancements.
- Assists in the determination of resident medical eligibility,
qualifying diagnosis, and Medicare or managed care eligibility
based on skilled services provided.
- Actively participates in QAPI (Quality Assurance Process
Improvement) program, taking a proactive approach to quality care
and services on a continuous basis with engagement of residents,
family members and staff.
- Participates in contributes to care planning meetings with
facility staff and family members.
- Follows the American Nurse Association's Scope and Standard of
Practice to ensure residents, families, and the public that the
nursing care they receive is competent and up to date with the
latest research; adheres to the Nursing Standards of Practice as a
benchmark for nursing care standards.
- Maintains resident confidentiality and privacy; treats
residents with kindness, dignity and respect; knows and complies
with Residents Rights rules; and promptly reports all resident
grievances to management.
- Understands and follows established Universal Precaution
guidelines, infection control and safety rules; ensures cleanliness
of assigned residents' rooms; displays an awareness of cleanliness
and proper maintenance at all drug carts/rooms and to maintain a
clean working environment; maintains a locked medication room and/
or cart when unattended; reports hazardous conditions or equipment
to the appropriate person; follows established safety rules, and
fire and disaster procedures.
- Communicates with family members regarding condition and
progress of residents, including in person and answering of
telephone inquiries taking into account the nature of the
information.
- Attends and participates in workshops and seminars to keep
current with changes in the long-term care industry as appropriate.
- Demonstrates high standards of excellence for meeting resident
needs. Communicates effectively, establishes rapport and maintains
relationships with residents.
- Meets requirement of regular attendance to perform job
duties.
- Will be with the on-call rotation. -
- Performs other duties as assigned. Qualifications: Education
and Experience
- RN graduate from accredited school of nursing (required).
- Knowledge of Medicare documentation requirements (required)
- Prior MDS experience (strongly preferred)
- 1 year of care planning experience in a long-term care
environment (preferred).
- Advanced preparation in geriatric care (preferred).
- Possesses the ability to make independent decisions when
circumstances warrant such action.
- Strong attention to detail and demonstrated ability to be
organized and multi-task in a fast paced environment.
- Demonstrates ability to work with a team and high collaboration
skills. License/Certifications
- State of Iowa RN or LPN licensure by the Iowa Board of Nursing
(required).
- CPR Certification (required).
