  • Job Requirements
  • American Heart Association certification is required for all direct patient care personnel.
  • Must include an image the front of card as well as an image of the back of the card.
  • Must be dated within 30 days of start.
  • For any position requiring a license or certification, attach scanned on-line verification page with a machine-generated date indication when the verification was performed within past 30 days.
  • Please ensure your uploading the state verification if available with NURSYS for RNs and LPNs.
  • Proof of vaccine Sept 2024 booster, declination or exemption(with agency letterhead).
  • Declination not accepted for CA or IL.
  • 9 Panel completed within 30 days of start date, preferred within 14 days of start.
  • Must be completed and resulted before start with MRO signature.
